Enrollment for your Hologic benefits is February 27 – March 11, 2022
Effective February 27th, Bolder Surgical employees will be transitioning to the Hologic benefits program. This Special Enrollment Period begins February 27th and ends March 11th. This is an active enrollment period which means that you need to take action between February 27th – March 11, 2022 in order to be covered effective February 27, 2022 for:
- Medical, including Prescription and Vision
- Health Savings Account when enrolled in the CDHP medical plan
- Dental
- Health Care and/or Dependent Care Flexible Spending Accounts
- Optional life Insurance
- Accident Insurance
- Critical Illness Insurance
- Pre-paid Legal Plan
This site provides you with everything you’ll need to be informed about your benefits. It includes a variety of resources, like the Bolder Surgical Transition Guide, plan summaries, comparison tools, and additional resources from our benefits providers. We encourage you to review the benefit materials to ensure that you are enrolled in the plans that work best for you and your family.
If you miss the enrollment deadline of March 11, you will not have coverage for the February 27, 2022, through June 30, 2022 period. For most benefits, once Open Enrollment closes, your only opportunity to make changes during the plan year is within 31 days of a “qualifying life event, such as a marriage, divorce, or the birth or adoption of your child. There will be an open enrollment held in May for a July 1 effective date, at which time you may change your elections.
